First of all, our waiter was outstanding. He was attentive and anticipated our needs before we had to ask for anything.
My cousins ordered fettuccini Alfredo with grilled chicken, and it was excellent. Creamy with no grittiness and no separation of butter or cream in the sauce. The chicken was juicy, tender, and cooked to perfection.
My eggplant parmigiana was lightly breaded and not even slightly oily. The sauce was beautifully seasoned and topped the eggplant without smothering it or leaving it dry. I really liked it and found it to be one of the best eggplant parmigianas that I have ever had.
My salad was one of the best dinner salads that I've ever had at a restaurant. The greens were very fresh and the tomatoes were actually flavorful and sweet- like garden tomatoes. Olives and shaved Parmesan, tossed with an amazing vinaigrette, finished this beauty. Oh yeah, not even a hint of preservatives on this salad.

Blah... ordered artichoke hearts and got hush puppies and they only have white napkins so it you are wearing black you'll have lots of white lint
Basically it seems anyplace serving food on river walk is going to be so-so

I would definitely not go back to this establishment. The food was cold and tasteless, and the service was slow and inattentive.
You know you're not fond of a place when the highlight was the bread and olive oil (which had pepper and shredded parmesan in it- yum).
I had the shrimp paesano. The shrimp were tasty, but the noodles and sauce were laughably bad. The noodles tasted instant, and the "sauce" was butter and a squirt of lemon. There was no hint of any other seasonings or salt. It was the same story for my companions' dishes. Bland, lukewarm spaghetti and meatballs. The spaghetti sauce tasted like tomato paste. The caesar salads were average and definitely nothing to write home about.
A note on the service: Our waiter wasn't friendly and did not write anything down. If a waiter does this, it's my expectation that they have the mind of a steel trap and don't ask what I ordered again after they leave the table to put orders in. Let me just say that my expectation was not met. The waiter brought the wrong wine bottle for my friends after telling them it'd be better to order a bottle, thinking they ordered the same, when in actuality one clearly ordered Chianti and the other ordered Cabernet. Neither was listening when the other ordered, so they were surprised when the waiter offered to bring a bottle. One friend ended up just compromising with what he brought because she didn't want to make a fuss.
I wouldn't recommend if you're looking for authentic, delicious Italian food or good service. 2 stars for an average salad, and the bread and oil. -

We have always been a fan of Paesanos, even after moving away from San Antonio 18 years ago. After many attempts over the years of getting Shrimp Paesano almost right, we decided to go get the real thing. We ordered the calamari appetizer, chicken Alfredo, spaghetti with meat sauce, and 2 orders of Shrimp Paesano. All the food was wonderful! The calamari was a huge portion that we almost couldn't finish and the Shrimp Paesano was even better than we remembered! Making it even better was the seat on the covered patio with a river walk view. The service was also outstanding. For special occasions, a date night, or a little self indulgent treat, you can't do better!

Shortly after landing in San Antonio, I was met by two things: extreme heat and my Uber ride. The latter was driven by a cool lady, who chatted about overcoming cancer and gaining strength through meditation. She even gave a food recommendation for Paesano's.
Hence next day for lunch, I eagerly headed over to this Italian restaurant below street-level on the Riverwalk. Paesano's lunch special came with a large bowl of house garden salad and bread to start. I particularly enjoyed dipping the slices of bread in the olive oil with cheese flakes. Then arrived their Shrimp Paesano. The lemon butter garlic sauce on spaghetti was creamy and quite heavy despite being just a lunch portion. The large bake-crusted shrimp was fresh and deliciously infused with the garlic butter; small wonder this was their house special.
I opted for their shaded patio seating which was equipped with ceiling fans to stay cool. The enthusiastic waiter was welcoming of visitors to the "Alamo City." Many locals here seem proud of San Antonio as being the birth place of the Lone Star State of Texas.
